Thank you for choosing
the Raritan Animal Hospital! Please choose
and fill out the appropriate forms
below and bring them with you for your visit
to help us get to know you and your pet. Please
plan to arrive 10 minutes prior to your scheduled
appointment to allow our staff the time to
create or update your pet’s record.
New Clients:
If you and your pet are new to our practice,
please complete the “New Client” Information
Form and print it out. Bring the completed form , proof of your pet's most recent vaccines, and a fecal sample with
you for your pet's first visit. We look forward to
meeting you and your pet!
New Pet:
Congratulations on your
new pet! If you are currently a client of our practice, please complete the "New Pet" information form and print it out. Bring the completed form, proof of your pet's most recent vaccines, and a fecal sample with
you for your pet's first visit. ( If you are a new client as well as a new pet owner, you need only fill out the "New Client" information form.)
Reptiles:
For owners of reptiles, please click on and
fill out the appropriate questionnaire
below. Print it out and bring it with you for
each
visit.
Birds:
For owners of birds, please click on and fill out the appropriate questionnaire
below. Print it out and bring it with you for each visit.
Small
Mammals:
For owners of small mammals, please click on and fill out the appropriate

Boarding:
Boarding is a special service we provide strictly
to our clients. This service is
especially useful to clients whose pets have
special medical needs. We will board cats
and
dogs (40lbs or
less). Please call ahead to book
your pet’s
boarding stay with us. Dogs should be up to date on their bordetella vaccine prior to boarding. Please complete
and print out the “Boarder Admission” Information
Form prior to dropping your pet off.
